Delivers personalized health management programs and resources that make it easy for individuals to participate in healthy behaviors.

Step 4: SUSTAIN
Encourages participants to maintain the positive lifestyle changes they have made and establishes a self-reinforcing feedback loop that promotes ongoing health improvement.
